Patient Forms, Map and Directions to Laramie Office
For your first visit to our office, we request that you come ten minutes early for your appointment. This allows us time to gather information (see link below to print, fill out and bring to your appointment) and for us to review your health history. If you have any medical issues (such as a need for dental premedication: artificial joint replacement within the past two years, heart conditions, or implanted device, etc), please be sure to let us know prior to your appointment. Also, any dental x-rays taken within the past two years could be helpful. You can have them mailed or emailed to us at firstname.lastname@example.org.
An examination will be done using multiple diagnostic tools that we determine are necessary for your unique circumstance. We utilize digital radiographs and digital photography.
